Original I-94 card 
Original letter from bank reflected in United States dollars in English. 
  ($18,000 = Applicant; $8,000 = Spouse; $4,000 = Each Child)
Letter explaining why a change of status is desired.
SEVIS Information Form. 
A $100 SEVIS fee payment receipt (can only be paid after admission and I-20 issuance)
